Generation Clinical Partners (GCP), a rapidly growing medical practice, is seeking an experienced (minimum 2 years) nurse practitioner or physician assistant to join our post-acute and long term care practice.

Job Description
The NP/PA serves as a critical member of the care team and will provide direct patient care, coordinate care transitions, and help lead quality improvement efforts in the skilled nursing facility setting. The focus of this position is to provide high-quality, cost-effective care to frail elderly patients, with a special emphasis on managing post-acute issues to prevent hospital readmissions.
Responsibilities
Within the scope of practice, effectively manage acute and chronic conditions by developing and overseeing high-quality, cost-effective treatment plans for patients in the facility, including ordering and interpreting diagnostic tests, prescribing medications, and other treatments as appropriate.
- Provide patient/family education and counseling in the areas of self-care, health promotion, maintenance, and disease prevention.
- Work closely with collaborating physicians and other colleagues, allowing for consultation, collaboration, and/or referral.
- Provide clinical leadership and education to nursing and other staff, contributing to facility performance improvement efforts.
- Document all medical evaluations, diagnoses, procedures, treatment, outcomes, education, referrals, and consultations according to established standards in GCP’s provider-friendly electronic health record.
- Analyze new knowledge gained from conferences, workshops, professional literature, or hands-on training and assimilate this knowledge into clinical practice.
- Maintain strict confidentiality with regard to protected health information and understand and adhere to GCP’s HIPAA Privacy and Security policy.
- Share in taking call with other GCP providers.
Qualifications and Skills
- Current valid and unrestricted RN and NP/PA licensure in Missouri.
- National Board Certification and MSN in a related field strongly preferred.
- 2+ years of experience, preferably in acute-care, long-term acute care, or inpatient rehabilitation practice settings.
- Demonstrated interpersonal skills to be approachable and understandable to patients, families, and staff.
- Ability to be flexible to meet the practice schedule to ensure appropriate coverage and high-quality patient care.
- Collaborative practice style and ability to work harmoniously with others.
- Demonstrated skill in resolving conflicts and promoting positive outcomes for patients.
